The Care Coordination Program is a resource for participants that will help them remain living healthy, safely, and independently in their home. There is no cost to participate in the program and it provides participants, their families, caregivers, and professional health care providers more flexibility to meet health care needs and improve care and quality of life for those enrolled. Predominately people age 55 and older, enrolled in Medicare and/or Medicaid, and typically living with certain disabilities and health issues that require some degree of daily living assistance may be eligible to enroll in the program.The project described was supported by grant number 1C1CM331042 from the Department of Health and Human Services, Centers for Medicare and Medicaid Services.
